• Church Growth Investment Fund Prospectus/Application
• Goldstar Trust Traditional IRA Application Form
• Investment Direction and Authorization Form
• Direct Rollover Transfer Form (If transferring account from another custodian) - Include a copy of your most recent statement
• Rollover Contribution Form (If transferring from another IRA and you have received the funds already) - Check should be made payable to GoldStar Trust Company

Roth IRA

• Church Growth Investment Fund Prospectus/Application
• GoldStar Trust Roth IRA Application Form
• Investment Direction and Authorization Form
• Direct Rollover Transfer Form (If transferring account from an existing Roth IRA custodian) - Include a copy of your most recent statement
• Rollover Contribution Form (If transferring from another Roth IRA and you have already received the funds) - Check should be made payable to GoldStar Trust Company
